TO support ‘Ask Your Pharmacist’ week, the medicines management teams at Northern Devon Healthcare Trust are asking patients to ‘tell us about your medicines’.

Medication errors can sometimes happen due to problems with communication, and so the teams are encouraging patients to tell them about the medicines they are taking as this will help patients get the most benefit from them.

Chief pharmacist Matt Kaye said: ’It is really important that patients give us as much information as possible about their medicines, including those bought in a pharmacy or supermarket. This includes telling us if you have any problems taking your medicines or if you forget or do not take any of your medicines.’

Patients are urged to bring all of their medicines into hospital with them, as well as their latest repeat prescription slip and any treatment cards or booklets they have been given with their medication.
